People who drive too close to the person in front, particularly tailgating. It can kill people, and also helps make traffic jams from nothing (due to overbraking, the 'shockwave' traffic jams). On top of that, it achieves nothing, pretty much.
I drove from NJ down to DC and then on into VA this last weekend, and saw some horrible interstate driving, a lot of pointless weaving and nearly everyone was driving too close to the person in front, particularly in the overtaking/outside lane; much of this driving would have embarassed an Italian. Driving back through Delmarva (where there are no interstates) the driving was much better, apart from the two dickheads tailgating me in their pickup signalling that I should move forward and tailgate the guy in front of me. But overall, much better. Do people generally get instruction on how to drive on interstates and other highways? Here in NJ, the driving test just involves driving around a carpark, so not great preparation for interstate driving (or, erm, any sort of driving on an actual road)(possibly, this is connected to the horrible lack of driving skills exhibited in NJ, second only to Indiana in my experience for horrible driving). In the UK, you learn and are tested on the equivalent of highways (two or more lanes divided by central reservation with 70mph speed limit) but not on motorways (which are more like interstates).
